The Universal referral form for scuba is an online tool which can be tailored to specific needs. It can be signed electronically, and you can also print the completed form. PADI Open Water Diver certification

A student diver must present the PADI Universal Referral Form for Open Water Diver Certification to a dive instructor. It states that the diver took initial training from another diver training organization. The form asks the instructor to instruct the student in open water diving, to sign a statement and to send it to the original instructor, who should certifiy the diver.

You can also use the Universal referral form for certification. However, PADI requires you to complete the course within 12 months of your last training. It is typically a two- to three-day course. You will be required to be able dive with your buddy at a maximum depth to 60 feet/18 meters.

Medical statement

You will need to fill out the Medical Statement for Universal Reference Form if you want to become a scuba diving instructor. This is an important document that will ensure you're as healthy as possible before you go diving. It is intended to ensure safety underwater. Please answer all questions honestly.


First, get your physician to sign the form. This is mandatory when you begin your training as an scuba diver. You will need a new medical release if you have any medical conditions. Lastly, you should always take a photocopy of your certification card.

Logbook

Before a diver is allowed to dive open water, they must sign a medical form. This usually takes place before a certification course starts. A doctor must sign the medical release. An updated medical release is necessary for patients with changing medical conditions. PADI provides a medical release. This medical release is equivalent in size to the PADI Liability Release/Assumption and Risk Form - General Training, or the EU version.

The universal referral form is different from the one used in a scuba logbook. It will list the open water dives and knowledge-development sections the student has completed. It also contains a signed statement from the instructor confirming that the diver has taken the required dives. The student then takes this form back to his or her original instructor who is supposed to certify him or her as a certified diver.

The course requirements

The Universal Referral Program covers most well-respected scuba certification bodies. This means that a diver can take the theory and pool part of his or her open water course with his or her local dive center but complete the open water check-out dives with a different agency. This is done with the cooperation of a dive center with a universal referral program such as SSI, NAUI, NASDS, YMCA, or other organizations.

PADI Open Water Divers requires that students pass the Knowledge Development as well as Open Water Dives sections. The student diver must also sign a medical waiver signed by their doctor before diving. The student diver must also provide a photo to accompany his or her certification card. Digital images can be accepted.
